Excursion & Camp Terms and Conditions

EXCURSIONS AND CAMPS GENERAL AGREEMENT:

By completing the College issued permission slip you:

  1. Agree to delegate my authority to supervising teachers. Such leaders may take whatever reasonable and appropriate disciplinary action they deem necessary to ensure the safety, well-being and successful conduct of the students/children as a group and individually.
  2. In the event of an accident or illness and contact with me being impracticable or impossible, I authorise the teacher-in-charge to arrange whatever medical or surgical treatment a registered medical practitioner considers necessary. I will pay all medical and dental expenses incurred on behalf of my child.
  3. Have provided all information necessary for supervising teachers to plan safe and reasonable health care support for my child. This includes, if relevant, information about any activity modifications my child may require for medical reasons.
  4. Consent to my child’s doctor or medical specialist being contacted by medical personnel in an emergency.


MEDICAL INFORMATION DISCLAIMER:

A copy of each student’s form must be taken on the excursion.The school collects the information contained in this form to provide or arrange first aid and other medical treatments for students. The information collected will be held at your child’s school and will be made available to staff or school and to medical or paramedical staff in the case of an accident or emergency.

Private transport consent

This form records consent for a student to travel in a private vehicle to a specific external activity. Where private transport is to be utilised, the school will ensure that any person who provides that transport is the holder of a current driver’s licence and that the vehicle they drive is registered. If the driver is not a parent of a child at the school or teacher from the school, the school will also ensure that the person holds a Blue Card or has completed the necessary Volunteers Induction.

Privacy statement

The school is collecting information about you and your child in order to obtain consent for the transportation of your child by private vehicle for the purpose of a kindergarten or school excursion. The information in this form may be given to drivers who will be transporting your child/ren in their vehicle. Your information will not be given to any other person or agency unless the school has obtained your consent, or the disclosure is required or authorised by law.

Activity risks and insurance

Please note that the Diocese of Toowoomba Catholic Schools does not have personal accident insurance cover for students. If your child is injured as a result of an accident or incident while participating in the activity, all costs associated with the injury, including medical costs are the responsibility of the parent/carer. Some incidental medical costs may be covered by Medicare. If you have private health insurance, some costs may also be covered by your provider. Any other costs must be covered by parents/carers. It is up to all parents/carers to decide what types and what level of private insurance you wish to arrange to cover your child. Please take this into consideration in deciding whether or not to allow your child to participate in this activity.
